Use of the Official Student Identification Card (ID)

The student identification card (ID) MUST be presented AT ALL TIMES in the following transactions:
- During Examinations. Faculty members are enjoined to see to it that students are wearing their ID's prominently, face forward, and that the person wearing the ID is the same as on the ID itself.
- Making payments to the cashier.
- Entering the library. The ID is to be presented to the personnel in charge of checking those who enter the library. It is also to be presented with the library card when borrowing books.
- Entering the campus. The ID is to be worn prominently, face forward, on entering the campus.
- Requesting for medical services. The ID shall be presented to clinic personnel when medical or dental service is required.
- Obtaining clearance or permit or student loans. Students must present the ID to the proper authorities, including the KASAMA, to have clearance signed, to obtain permit for any campus activity, or to obtain student loans from the Department of Student Affairs.
- On officially sanctioned field trips, ID’s should be worn at all times on entering the offices or premises of other agencies during approved field trips/educational tours.
SANCTIONS
There shall be NO exceptions or exemptions from compliance with this directive. All personnel in authority in the offices involved are directed to set up proper procedures to ensure compliance, if none have already been established.
Loss of ID card shall be reported to the Registrar and to the Security Office within thirty six (36) hours of such loss, or face disciplinary action.
